Service Eligibility

• Pennsylvania Resident
• 18-59 years of age
• Physical Disability that is expected to last more than 12 months including but not limited to:

Traumatic brain injury, diabetes, chronic arthritis, chronic asthma, heart/kidney problems, hypertension, cancer, stroke, paraplegic, multiple sclerosis, lupus, Parkinson’s disease, Huntington’s disease, cerebral palsy, epilepsy, respiratory disorders, pulmonary disease, or amputation as a result of muscular dystrophy, congenital, injury, etc.
